If you have attended a consultation or information session with Dr Sally or her team and have expressed a wish to proceed, you will have been sent a consent form (check spam folder too!).

Please read it carefully and contact us if you have any further questions or need a form sent again. When you are happy that all your questions have been answered, please sign and submit the form.

Once you have signed the consent form, please complete the registration form below.

You will need to provide photographic evidence of your height and weight unless you have had a face-to-face consultation with us.

Dr Sally takes her professional obligations seriously and will only issue medication to those who are eligible so that she can provide safe and effective care, as per her medical responsibilities .

Once your consent and registration forms have been approved by Dr Sally Norton, she will issue a prescription.

This will either be for delivery to your home as soon as your online purchase has been made.

Or, alternatively, she will issue a prescription to a local pharmacy of your choice if you have identified supplies and provided the pharmacy address and contact details on the registration form. You will pay the pharmacy for your prescription when you collect.
